Who We Are
A Small Agency With a Big Heart
Live Well MN LLC was born out of a deep commitment to the people of Minnesota. Founded in 2020, we set out to build something different — a small, family-oriented agency where every client is known by name, not a case number.
Over the past 6 years, we've grown steadily by doing one thing well: showing up for the people who need us. We work alongside individuals with disabilities, seniors, and families navigating Minnesota's complex care system — helping them access the services they're entitled to and the support they deserve.
We are proud to be an enrolled 245D provider, serving the Twin Cities and Greater Minnesota with home and community based services and PCA/CFSS. Our team reflects the communities we serve — bilingual, culturally responsive, and deeply invested in the wellbeing of every person in our care.

Owner & Designated Manager
Isaak founded Live Well MN LLC in 2020 with a vision to bring compassionate, personalized home care to Minnesotans. As Owner and Designated Manager, he oversees all operations and ensures every client receives the highest standard of 245D-compliant care.
