Personal Care Assistance (PCA)
Hands-on support for daily living
Personal Care Assistance helps individuals with physical or cognitive disabilities manage the tasks of daily life — so they can remain safely and comfortably at home. Our PCAs are carefully screened, trained, and matched to each person they serve.

Eligibility
PCA services are available through Minnesota Health Care Programs (MHCP) for individuals who qualify based on functional need. Our team can help you determine eligibility and navigate the enrollment process.
Minnesota Waiver Services
Long-term support through state programs
Minnesota's Medicaid waiver programs provide funding for long-term services and supports that help individuals with disabilities or complex needs live in their community rather than a facility. Live Well MN LLC is an enrolled provider for multiple waiver programs.
